The role of new pro-inflammatory and/or pro-fibrotic molecules in the pathogenesis of systemic sclerosis.

Introduction: Systemic sclerosis (SSc) is a generalized connective tissue disease affecting the skin and internal organs. The pathogenesis of SSc is characterized by inflammation, vasculopathy and fibrosis. To date, none of the tested drugs have demonstrated convincing efficacy in the treatment of SSc. S100A4 is involved in the regulation of cell motility, proliferation, apoptosis, angiogenesis and remodeling of the extracellular matrix. It was originally described as a promoter of metastasis in tumors, however, its pro-inflammatory properties have recently been demonstrated in inflammatory rheumatic diseases. The aim of this study was to assess the role of S100A4 in pathological activation of fibroblasts in SSc and in experimental models of dermal fibrosis. Results: The expression of S100A4 was increased in the skin of SSc patients, in SSc fibroblasts and in experimental fibrosis in a TGF-β / Smad dependent manner. Overexpression of S100A4 or stimulation with recombinant S100A4 induced an activated phenotype in resting normal fibroblasts. In contrast, inhibition of S100A4 or its complete deficit abrogated the pro-fibrotic effects of TGF-β and decreased the release of collagen. Quality of life with scleroderma

This bachelor thesis deals with the evaluation of the quality of life with scleroderma, a chronic autoimmune disease. The work is focused on determining the health-related quality of life of patients with localized and systemic scleroderma. The values obtained are then compared with the quality of life of the healthy population and with the health-related quality of life of the patients with selected diseases (psoriasis, systemic lupus erythematosus, rheumatoid arthritis and breast cancer). The research was conducted in the form of a randomized SF-36 questionnaire, a globally used tool for assessing the quality of life in chronically ill patients, which evaluates 8 areas of life. The results of the SF-36 questionnaire were further evaluated by T-test to determine statistically significant values. This questionnaire was further extended by supplementary questions. The results show that patients with scleroderma do not achieve the same quality of life as a healthy population in all evaluated areas. Patients with scleroderma rate their quality of life worse than all selected diseases (except for patients with localized scleroderma compared to those with systemic lupus erythematosus). Autor: Jana Martiňáková Title: Case study of physiotherapy treatment of a patient with systemic scleroderma Objective: The main aim of this thesis is a summary of theoretical informations about the diagnosis of systemic scleroderma, case reports of physiotherapy treatment of a patient with this diagnosis and evaluation of the effect of therapy. Methods: The work is divided into a general and a special part. The general part deals with the distribution of scleroderma, further analyzes the problem of systemic scleroderma with a focus on physiotherapy treatment and physical therapy. Special section includes a case study of physiotherapy treatment of a patient with this disease. The part of the special section are initial examination, setting goals and treatment plan, therapies, final examination and evaluation of the effect of therapy. This case study was created during continuous professional clinical practice held from 5. 1. till 30. 1. 2015 in Revmatologický ústav in Prague. Results: After performed therapies were achieved positive results especially during walking, improved respiratory stereotype, increase joint mobility acres upper limbs and improve grips.
